Within Hatzfeld care older age dementia services, we specialise in working with people who experience organic and functional disorders. The team’s purpose is to provide a service in a caring, supportive, and holistic manner to improve the lives of people who are experiencing problems with cognition.

We aim to support those diagnosed with Dementia to live well with this diagnosis and promote a “Living Well with Dementia” focused approach. We offer a responsive and compassionate care service for individuals facing illness, emphasising high-quality care underpinned by the principles of positive behaviour support.

Our interventions follow a safety plan that aims to transform culture and leadership, ensuring safe and effective care. Grounded in the Model of Care methodology, our person-centred approach is tailored to the unique needs of those with dementia.

We collaborate closely with local authorities and mental health teams to provide comprehensive assessment and treatment, employing psychosocial interventions such as evidence-based therapies, behaviour management, and pharmacological/non-pharmacological approaches. Our clinical lead, a registered mental health nurse, oversees all interventions.
